                                2006-2007 Directory Form

Tentative deadline for the Directory Form will probably be sometime in late October

If you would like to be in the 2006-2007 Burlington County YMCA Directory, please fill out the directory form below and submit your information.  If you submitted your info last year and everything is the same, you do not have to submit anything.  If one item changed (phone number or e-mail address), then just include that change and your name.  This directory is just for distribution amongst the Hurricanes family.  We've included space for phone numbers and e-mail addresses for everyone.  If both parents have the same phone # and e-mail, one entry is fine.  If there are entries that you wish to leave blank, that's fine as well.  Also, if there are different family names for parents and swimmers, include that in the parent's first name section or include a note in the comments section at the end.  In addition to the alphabetized list, we'll try to include a page or two with lists of the swimmers in each age group to make it a little easier to track down the names of swimmers and parents.  Thanks in advance for your help.
